Community Pharmacy Diabetes Programs
Diabetes patients visit pharmacies more often than any other medical setting - so what happens when community pharmacies use their skills to help those with diabetes manage their disease?
A new report from the California Healthcare Foundation that profiles eight community pharmacy-based diabetes care programs tells us. "These geographically-diverse programs serve a wide variety of patients - rich and poor, rural and urban, English-speaking and primarily non-English speaking. All eight are aimed at improving patient outcomes and reducing costs related to complications of a chronic disease, which means they could be reproduced elsewhere."
