The Innovation Network is a nonprofit organization working to share planning and evaluation tools and know-how. They've complied a wealth of free on-line resources for non-profit organizations. Their Tools and Resources require registration (free), but you can sample a few before you sign up. You'll find guides for planning, evaluation (including surveys, interviews, focus groups and more), and capacity building from other non-profits who have created successful models.
